Methodology
Reproducible snapshots, never guessed rates.
The current dataset was generated on August 23, 2026 from eight official workbooks published by Illinois, Mississippi, and West Virginia. It compares HCPCS Level II rows across stated historical periods and preserves the published amount, modifier or rate basis, effective period, and official source page.
What is included
Only non-dental HCPCS Level II identifiers and the minimum fields needed to reproduce a rate comparison are published. Descriptions are intentionally omitted. An “added” or “removed” row describes a workbook difference; it does not independently establish coverage, termination, or reimbursement eligibility.
How failures are handled
The release checks exact row totals, known comparison samples, source-file fingerprints, allowed code format, and the absence of restricted descriptions. If a workbook cannot be parsed or the release checks fail, publishing stops. The system does not fill gaps or infer a rate.
Freshness boundary
The public explorer begins from source workbooks reviewed August 23, 2026. Automated checks retrieve the current official state files on a schedule, compare normalized rate keys, and alert only after a validated promotion. This is not a real-time agency feed. A missing, malformed, or unverifiable source marks that state stale and suppresses its alerts instead of guessing.
Verify before acting
Rates and policies can change after a snapshot. Confirm the current official fee schedule and applicable state policy before billing, purchasing, contracting, or making a coverage decision.
